Symptoms

On a Plesk server with WP Toolkit, an unwanted prefix is added to the name of a cloned database.
The option seems to be hardcoded and cannot be changed.

Cause

The option "Add a prefix and underscore to the beginning of database names" is enabled at Tools & Settings > Database Hosting Preferences

Resolution

Log into Plesk
Go to Tools & Settings > Database Hosting Preferences
Set the option "Add a prefix and underscore to the beginning of database names" to Off.
