Unable to start Apache: You must load mod_request to enable the mod_auth_form

Symptoms

Apache fails to start with the below error:

You must load mod_request to enable the mod_auth_form functions
AH00016: Configuration Failed

Cause

request module is not enabled in the server

Resolution

1. Log in to Plesk UI

2. Go to Tools & Setting > Apache Web Server and enable request module.
