Answer

Recommendations on how to prevent authentication issues are:

  • Double-check the 6-digit code to ensure its acceptance.
  • Verify the correct MFA code source is being used, for example when many different Google Authenticators are set simultaneously then match the appropriate one for the attempted login.
  • Ensure that the mobile device has its time sync function enabled due to the importance of the timestamp for 2 factor authentication methods (2FA). More on this here.
  • Check that system time on the server is also in sync by reviewing the systemd-timesyncd or chronyd services status or setting this from Plesk as instructed here.  
  • Avoid exceeding the limit of consecutive failed authentication attempts ( by default 5 ) to avoid a fail2ban for a few minutes.
