1. The customer has a subscription with many subdomains;
2. Using the File Manager customer creates a directory under the root directory of the subdomain;
3. As a result, the directory was created under the root directory of the subscription.
This is a known bug PPP-27075 which is already fixed in Plesk Onyx 17.0.
Upgrade Plesk or migrate the affected customer to the upgraded Plesk.
